Jacquelyn "Jackie" Simmons, 34, of Ghent, Carroll County, KY, died on Friday, May 8, 2015 at her residence in Ghent, KY.
She was born January 25, 1981 in Carrollton, Carroll County, KY, the daughter of Thomas D. Simmons and Judy K. Forwood Simmons of Ghent, KY. She was a graduate of Carroll County High School and attended Gratz Seventh-day Adventist Church in Gratz, KY.
She is survived by her parents of Ghent, KY, two sisters, Peggy Dickerson of Worthville, KY, and Mary Dayton of Maryville, TN, two brothers, Paul Simmons of Ghent, KY, and Harry Mercer of Sulphur, KY, along several nieces and nephews.
She was preceded in death by her brother, Christopher S. Simmons on March 8, 2012.

In lieu of flowers, memorial gifts may be given to the Jacquelyn Simmons memorial fund in care of the funeral home.
Visitation
Visitation will be from 11:00 A.M. - Noon on Monday, May 11, 2015 at the funeral home.
Funeral
A Memorial Service will be on Monday, May 11, 2015 at Noon at the Tandy-Eckler-Riley Funeral Home with Pastor Lin Powell officiating the service.
Interment
Cremation has been chosen by the family.
